Records Release Form

After you complete the enrollment, you may request other Healthcare Practices to send your medical records (labs, radiology, immunization records, etc) to us

  1. Download the Records Release Form, enter your information, and send to the Healthcare Practice you wish to request records from.

  2. Request the Healthcare Practice to send the records to Dr. Neil using the contact information found on the Records Release Form.
