Annual Exam House Call Concierge Medicine

  • During the Annual Health Exam visit, Dr. Neil will perform a thorough physical examination and collect a detailed health and lifestyle history. A variety of medical tests will be conducted, and specialty lab tests will be recommended if indicated.

    In preparation for your first visit, start collecting the following information:

    • Preferred pharmacy

    • Preferred laboratory

    • Preferred Radiology facility

    • Nearest urgent care

    • Preferred hospital/emergency room

    • Specialists contact information

Virtual Health Telemedicine Concierge Medicine Same day appointment

  • These are telemedicine visits.

    Duration: 15-30 minutes.

At-Home Labs : We come to you for Mobile Labs. Drop off to your preferred lab including Quest Diagnostics, LabCorp, Genova Diagnostics, Access Medical Labs, Accureference, and more!

    • Book at-home lab appointments.

    • A lab technician comes to you to collect your labs and delivers the specimens to your preferred laboratory.

    • Once testing is completed, you receive your results via video consultation.

Medical Records

After you complete the enrollment, you may request other Healthcare Practices to send your medical records (labs, radiology, immunization records, etc) to us

  1. Download the Records Release Form, enter your information, and send to the Healthcare Practice you wish to request records from.

  2. Request the Healthcare Practice to send the records to Dr. Neil using the contact information found on the Records Release Form.
